• 63-year-old female presenting with sudden onset of shortness of breath and severe difficulty breathing during air travel
• Difficulty walking stairs, needing to stop every 10-20 yards due to breathlessness
• Denies shortness of breath at rest, cough, wheezing, bilateral lower extremity swelling, or fever
• No palpitations, chest pain, or excessive sweating reported
• Medical history includes hypertension, chronic obstructive pulmonary disease (COPD), anxiety, and depression
• Past smoking history starting at age 19 for 17 years
• Patient appears stressed and anxious upon examination
• Vital signs show elevated blood pressure at 155/90 mmHg, respiratory rate of 26 with labored breathing
• Oxygen saturation significantly low at 70%
• Auscultation reveals an S4 gallop heart sound, clear lung fields without wheezing or rales
• No bilateral lower extremity edema observed
