- Complains of insomnia, nervousness, and fatigue progressing over past few months
- Reports occasional palpitations, heat intolerance, and unintentional weight loss despite increased appetite and food intake
- Family history of maternal grandmother with “lazy thyroid”
- Physical exam findings: tachycardia, lid lag, mildly enlarged, soft, nontender thyroid with palpable isthmus
- Additional signs include hyperreflexia and fine hand tremor, indicating possible thyroid dysfunction such as hyperthyroidism
