• 65-year-old female with no significant past medical history
• Progressive insomnia, nervousness, and fatigue over several months

Additional symptoms:

• Occasional palpitations
• Heat intolerance
• Unintentional weight loss despite increased appetite
• Family history of the maternal grandmother with “lazy thyroid”

Physical exam findings:

• Tachycardia
• Lid lag
• Mildly enlarged, soft, nontender thyroid with palpable isthmus
• Hyperreflexia
• Fine tremor in hands
